I confirm that I have not been suffering with any symptoms of COVID-19 such a high temperature, new continuous cough, loss of taste or/and smell in the last 14 days.

I confirm that I have not been in contact with anyone who has displayed any symptoms of COVID-19, or anyone who is currently self isolating with symptoms (confirmed or not) of COVID-19 in the last 14 days.

I confirm that I do not have any bloodbourne pathogens, transmittable diseases or recent illnesses. (It's okay if you do, we just want to know for our staff and other client's safety).

I confirm that I do not have diabetes, epilepsy, hemophilia, a heart condition, nor do I take blood thinning medication.

I confirm that I do not have any other condition that may interfere with the application or healing of this tattoo.

I am not the recipient of an organ or bone marrow transplant or, if I am, I have taken the preventive anti-biotics.

I confirm that I do not have a mental impairment that may affect my judgment in getting this tattoo.

I confirm that I am not currently pregnant or breast-feeding.

I confirm that I understand the risks associated with getting a tattoo. I fully understand that these risks can lead to (but is not limited to) infection, scarring, difficulties in detecting melanoma and allergic reactions to tattoo pigments/ink.

I confirm that I will follow the aftercare advice that is given by my Artist and understand that if I don't follow this advice then my tattoo may not heal to it's full potential. I understand that the care and cleanliness of the tattoo is down to myself once I leave the studio. If any touch-up work to the tattoo is needed due to my negligence, I agree that the work will be done at my own expense/at tattooist's discretion up to 6 months after your tattoo.

I confirm that only I am responsible for the spelling, symbolic meaning and/or orientation of my tattoo. Once presented with the final design of my tattoo, I agree that it is my sole responsibility to check; any spelling errors, the piece isn't upside down or flipped etc.

I confirm that I will not video record or distract the staff while they are working as this violates personal safety for both myself and my Artist.

I confirm that I am not under the influence of alcohol or drugs.

I confirm that I understand the importance of having increased blood sugar levels for a tattoo appointment by eating or having a sugar-y drink up to 4 hours prior to my appointment.

I confirm that if I can't make my appointment, I will let the staff at The Freakshow know, so that they can offer my appointment slot to another client. I also understand that the deposit that I have paid is non-refundable and non-transferable, so if I can't make my appointment, I will need to pay a new deposit for my new booking.
